School Districts and Libraries

The Phase 1 Online Questionnaire is now available for school districts and libraries. Please note the questionnaire is due October 15, 2010. Also, the survey is a single page this year, so if you need to come back to it, simply print the survey in order to gather data, then come back to the survey when you are ready to submit it.
