North Carolina Charter School Information

The Phase 1 Online Questionnaire is now available for charter schools. Please note the questionnaire is due the week of October 15.. Also, the survey is a single page this year, so if you need to come back to it, simply print the survey in order to gather data, then come back to the survey when you are ready to submit it.

If you need to conduct a household income survey (because your school does not participate in NSLP), you can download a MS Word template survey by CLICKING HERE. Instructions for using the template can be found HERE. Right-click both links and save them to your hard drive. You may then modify the introduction to the template as needed before you send it to parents. Be certain to place it on SCHOOL LETTERHEAD.

REMEMBER: If you conduct the survey, you must have a 50 percent or greater return rate (based on the student population/ADM) and have them returned to us by November 21. If you have difficulty obtaining that many by that date, please contact us.
